Accessing the Readmissions Section
To access the Readmissions section, select the Outcomes Module from the left side of the screen. Readmissions is the third section from the top. Click the arrow on the right side to expand and view this section. It features three visualizations that chart relevant admissions and discharges.
30-Day Readmission Rate and Volume
The 30-day Readmission Rate & Volume graph depicts the number of inpatient admissions within 30 days after a prior discharge. Additionally, it displays the readmission rate, calculated as the count of readmissions divided by the count of admissions, enabling the evaluation of pre-intervention baseline and post intervention performance. Hovering over the graph will highlight a specific section of a bar graph that provides a tooltip with the total count of readmissions and percentage rate.
Note: Caution is advised for recent months as some admissions may not yet be discharged, potentially inflating the readmission rate.
Readmissions by Anchor Discharge RI
The Readmissions by Anchor DIscharge RI graph illustrates the rate of inpatient admissions occurring within the 30 days after a prior discharge based on the RI groups. The RI groups will use the visit's last valid RI score generated during the anchor inpatient encounter. Comparing readmission rates across different RI groups helps in identifying opportunities to reduce admissions. Hovering over the graph will highlight a specific section of a bar graph that provides a tooltip with readmissions rate and percentage of the RI group.
Note: Caution is advised for recent months due to potential ongoing admissions which might inflate the readmission rate.
Readmissions by Anchor DIscharge Unit
The readmissions by Anchor Discharge Unit graph facilitates a comparison of readmission rates across units, aiding in the evaluation of opportunities for reducing readmissions. The graph is initially limited to 10 units displayed, allowing for the user to select/deselect the units to graph. Hovering over the graph will highlight a line providing the readmissions rate for that unit and time period.

Below the three Readmissions visualizations is the patient list, which includes the following patient information:
- Visit number
- Medical Record Number
- Patient name
- Admit date
- Discharge date
- Facility patient is located
- Service line code
- Discharge list
- Discharge unit type
- Discharge disposition
- Admit RI score
- Minimum RI score
- Maximum RI score
- Exact LOS
- LOS calendar
- LOS midnights
- Admitting provider
- Attending provider
- Primary diagnosis
- Unplanned transfer count

This section displays a comprehensive list of patients and their data featured in the Readmissions visualization. In the top left corner, users can search through this list, while also having the option to download it in Excel or CSV format on the top right side of the screen. The bottom left corner offers controls to adjust the number of patients shown per page, with choices of 10, 25, or 50. Additionally, navigation through the patient list is facilitated by front and back arrows located in the bottom right corner.
Patient RI Graphs
Below the Patient List, the user will find a set of RI graphs corresponding to each patient. The number of graphs displayed is determined by the 'Items per page' setting selected in the patient list above.
Users can customize the display at the top of the RI graphs by selecting the Chart Title menu. The options available for customization include:
- Visit number
- Admission date
- Medical record number
- Patient name
- Full location
- Unit & room
- Room number
- Anonymous
Users can click on a patient’s graph to open it in full-screen mode for a larger view. Alternatively, hovering over a patient’s graph will provide a concise preview of the patient’s visit information.
