Patient MEWS
A clinical care assessment system utilizing Heart Rate, Respiration Rate, SpO2 Percentage, Temperature, NIBP, and AVPU to determine a patient evaluation score. Functioning as a prediction system, MEWS recognizes early signs of health deterioration to ensure a timely escalation of patient care.
Clinical Assessment of Consciousness

Patient is awake and aware of environment. Responds cognitively to questions and commands. |

Patient is not fully awake but responds to verbal stimulation and commands. |

Patient is difficult to awaken and only responds to painful stimuli. |

Patient is unresponsive and does not respond to any stimulation. |
Updating Patient MEWS Assessment
- Access the Dashboard Tab.
- Select a patient from the My Patient’s Tab.
- Select the + on the bottom of any patient page.
- Select the MEWS icon.
- Click the Sync button to pull in data from the patient’s latest vitals within SafeNSound.
If latest vitals are not available to display, manually enter the following:
- Heart Rate
- Respiration Rate
- SpO2
- Temperature
- Blood Pressure
- AVPU- Alert, Voice, Pain, Unresponsive
- Select Update

NOTE: If a patient’s MEWS Assessment is actively declining, the nurse is encouraged to contact an attending physician before escalation is necessary.
