Patient MEWS

A clinical care assessment system utilizing Heart Rate, Respiration Rate, SpO2 Percentage, Temperature, NIBP, and AVPU to determine a patient evaluation score. Functioning as a prediction system, MEWS recognizes early signs of health deterioration to ensure a timely escalation of patient care.

                                     

Clinical Assessment of Consciousness

A

Patient is awake and aware of environment. Responds cognitively to questions and commands.

V

Patient is not fully awake but responds to verbal stimulation and commands.

P

Patient is difficult to awaken and only responds to painful stimuli.

U

Patient is unresponsive and does not respond to any stimulation.

Updating Patient MEWS Assessment

  1. Access the Dashboard Tab.
  2. Select a patient from the My Patient’s Tab.
  3. Select the + on the bottom of any patient page.
  4. Select the MEWS icon.
  5.  Click the Sync button to pull in data from the patient’s latest vitals within SafeNSound.

                                       

If latest vitals are not available to display, manually enter the following:

  • Heart Rate
  • Respiration Rate
  • SpO2
  • Temperature
  • Blood Pressure
  • AVPU- Alert, Voice, Pain, Unresponsive
  • Select Update

Perform Observation Every 2 Hours

Perform Observation Every 1 – 2 Hours

4-5

Perform Observation Every 1/2 Hour

 

NOTE: If a patient’s MEWS Assessment is actively declining, the nurse is encouraged to contact an attending physician before escalation is necessary.
