Workflow
As a patient is admitted into the ED, a clinician will perform an initial assessment of the patient. It is during this assessment that the clinician will utilize the RI Triage module to obtain an estimated risk level.
Patient Selection
The clinician will log into RI Mobile and will land on the Patient List view. If this is the clinician's first time accessing the RI Triage module, they will need to setup the filter view to see the Risk Level patient list as opposed to the Warning patient list. This is accomplished in the filter menu. Once the user has applied the filters, the patient list will show patients with their current estimated risk level.
If the user has not made any changes to their filter selections prior to exiting or logging out of the application, the same filter selections will be displayed when the user logs back into the RI Mobile application and they'll see the RI Triage patient list view.
Once the Risk Level filter is applied, the clinician will either find the patient in a 'Risk Unavailable' card, with no color or score, or can perform a patient search through the search icon. If the ADT has not yet been received for the patient's ED admission, the patient will not be available in RI Triage module. The clinician will need to wait until the patient has been received into the RI Mobile to complete the Risk assessment module.
Once the patient is found through the 'Risk Unavailable' card or the patient search, the clinician will open the single patient view. The single patient view will display the 'No Risk level has been generated' page with the edit button enabled to add a New Assessment.
Triage Assessment Entry
The clinician will enter a new assessment by clicking the "notepad/pencil" icon and select the assessment button.
The enter menu will display the configured entry options of Assessments or Notes/Interventions for the user. Selecting Assessments will open the page to provide the COVID classification and the results of the patient's nursing assessment. In the top section of the assessment entry page the clinician will enter in the patient's COVID classification, COVID or non-COVID.
In the bottom section of the assessment entry page the clinician will enter in the results of the 11 nursing assessments. The clinician will select 'Met' or 'Not Met' for each separate nursing assessment.
The clinician will be required to answer all nursing assessments to be allowed to submit the entry for an estimated risk level calculation. The clinician should follow hospital procedure on answering 'Met or 'Not Met' if the assessment was not completed. Once the clinician has submitted the nursing assessment =, ia all the required data is available, the RI server will calculate the RI score and estimated risk level returning the results to the single patient view.
If the clinician does not enter an answer for all 11 nursing assessments and the COVID classification, the 'Submit' button will remain 'inactive' until the user completes all entries. This is to ensure the RI Server has all of the required data to calculate an estimated Risk level.
Selecting the 'X' will exit the Assessments panel. If the user selects the 'X' without savings after entering in all assessment entries the user will be prompted to confirm the close action without saving their entries.
Filtering
When RI Triage is enabled, the Patient List view can be filtered by the different estimated Risk Levels of High, Medium, Low or Risk Unavailable. For a user to use the RI Triage module, they will need to select one of the filter choices of High, Medium, Low or Risk Unavailable for Risk Level selection. The user will need to deselect all 'Warning Level' filters for patient warnings and to be able to select any 'Risk Level' filter in the Filters menu. A user cannot select 'Risk Level' filters and 'Warning Level' filters at the same time.
Both Warning level and Risk Level filter options will display when neither have any choices selected. Once a user selects a Warning Level filter a message will replace the Risk Level filter options, instructing the user to deselect all Warning Level filters to be able to see the Risk Level filter options to access the estimated Risk Level list view.
The same will apply in reverse when any Risk Level filters are selected a message will replace the Warning Level filter options to instruct the user to deselect all Risk Level filters to be able to see the Warning Level filter options and access the Warning Level list view.
