As a patient is admitted into the ED, a clinician will perform an initial assessment of the patient. It is during this assessment that the clinician will utilize the RI Triage module to obtain an estimated risk level. 

Patient Section 

The clinician will log into RI Trend to access the RI Triage module. If this is the clinician’s first time accessing the RI Triage module, they will need to set up their preferences to see the Risk Level view as opposed to the Warning Lane view. This is accomplished in the Filter menu. Once the filters are applied to show Risk Levels the user can save this filter selection as a Preset. 

If the clinician has previously accessed the RI Triage module in RI Trend and saved a Risk Level Preset, this preset can be selected to return to the Risk Level view quickly. The last filter used during the clinician’s last session is the filter applied by default upon logging in.

Once the Risk Level filter is applied, the clinician will either find the patient in the ’Risk Unavailable’ lane or can perform a patient search through the search icon. If the ADT has not yet been received for the patient’s ED admission, the patient will not be available in RI Triage module. The clinician will need to wait until the patient has been received into RI Trend to complete the Risk assessment module.

Once the patient is found through the ‘Risk Unavailable’ lane or the patient search, the clinician will open the patient specific view. The patient specific view will display the ‘No Risk level has been generated’ message on the page with ability to add a New Assessment.

Triage Assessment Entry

The clinician will click the New Assessment button that will open a side panel to provide the COVID classification and enter in the results of the nursing assessment. In the top section of the assessment side panel the clinician will enter in the patient’s suspected COVID classification, COVID or non-COVID.

In the bottom section of the assessment side panel the clinician will enter in the results of the 11 nursing assessments. The clinician will select ‘Met’ or ‘Not Met’ for each separate nursing assessment. The clinician will be required to answer all nursing assessments to be allowed to submit the entry for an estimated risk level calculation. The clinician should follow hospital procedure on answering ‘Met’ or ‘Not Met’ if the assessment was not completed. Once the clinician has submitted the nursing assessment, if all of the required data are available, the RI Server will calculate the RI score and estimated risk level returning the results to the patient specific view.

The clinician is required to enter in an answer to all 11 nursing assessments. The side panel will not provide an enabled ‘Submit’ button unless the classification and all 11 nursing assessments are answered. If the hospital has the Notes/Interventions feature enabled, the clinician will be presented a pop-up warning reminding the user that the classification and all nursing assessments are required.

Prior to submitting a completed assessment the user will be prompted to review the entries to be sure they are correct. Assessments cannot be modified, but new assessments can be added.

Selecting the “X” will exit the Assessments panel. If the user selects “X” without saving after entering in any assessment, or COVID classification, the user will be prompted to confirm the close action without saving their entries.

 

When the data is not current or available to calculate an estimated risk level the RI Server will return a message to explain why the risk has not been calculated. 

Once all the data is received and entered, the RI Server will calculate the patient’s RI Score and determine an estimated Risk level to the Patient Specific View page. From here, a clinician will be able to click on the bar graph to see the vital and nursing assessment details that were used for the Risk level determination. If the data are not yet available for display, the cursor will provide a tooltip of “Details Pending”.
Once the details are available, the user can click on the bar graph to open the Details side panel.

Filtering 

For a user to use the RI Triage module, they will need to deselect all “Warning Level” filters for RI Trend and select any “Risk Level” filter in the filters menu. A user cannot select “Warning Level” filters and “Risk Level” filters at the same time. A warning message will be displayed if a user tries to select both “Warning Level” and “Risk Level” filters at the same time.

The array view of risk lanes can be filtered by the different estimated risk levels of High, Medium, Low or Risk Unavailable.
