Clicking on a graph within the Array View or on a patient’s warning card within the Warnings notification screen launches the Patient Specific View within a new window. The resulting display is an enlarged view of a single patient’s graph from which users can see the individual patient’s RI scores over different timespans, and access clinical inputs to each RI score, as well as patient visit and demographic information, and warning details. RI Trend graphs are accessible via integration with an organization’s EHR which provides seamless access to patient clinical data. Please refer to the Integration section of this guide for more information. 

Patient Banner 

The graph’s header provides details of the patient’s current admission. The banner includes the following patient information:

  • Current RI Score 
  • Current Warning State
  • Patient Name
  • Patient Age
  • Gender
  • Medical Record Number
  • Visit Number
  • Date of Birth 
  • Admit Date
  • Provider 
  • Diagnosis 
  • Facility
  • Unit, room, bed
  • Discharge Date (visible when viewing a prior visit with RI Score)

Within the “Diagnosis” field, if more than one diagnosis is available, the first diagnosis will display only. Hovering over the ellipsis within the Diagnosis field provides a tool tip of additional diagnoses available for the patient.
